FOLLOW THE STEPS FOR A REGISTRATION

STEP ONE

Prerequisite:

To start the registration process with MiLAT, the applicant must be between the age 16-19 years and have completed AT LEAST THREE (3) YEARS OF SECONDARY SCHOOL EDUCATION.

STEP TWO

Gather your documents:

Four (4) passport size photographs of the applicant.

One (1) copy of the applicant’s Parent or Guardian’s valid Trinidad and Tobago Identification Card.

One (1) copy of the applicant’s Identification Card/Passport.

One (1) copy of the applicant’s valid Trinidad and Tobago Birth Certificate.

One (1) copy of the applicant’s Immunization Card.

One (1) valid Transcript from the applicant’s last secondary school.

Police Certificate of Character. For assistance you can refer to:  Obtaining a Police Certificate of Character.

National Insurance Number (NIS) obtained from the National Insurance Board (NIB)

One (1) valid bank account on the applicant’s name. No joint account.

STEP THREE

Complete the Registration Form:

Download the Application Form (letter; paper size 8 1/2′ x 11′). The medical section of the Application Form must be filled out by your Physician!

Download the Behavioural Matrix Form (letter; paper size 8 1/2′ x 11′). This form must be filled out by the applicant’s last secondary school.

You can also get assistance with the Registration Form by visiting the MiLAT Human Resource Office, in Mausica, or any of the ttconnect office locations across Trinidad and Tobago.

STEP FOUR

Drop off your Package:

Please ensure that you have all of your documents from Step Two.

Make sure the Application Form, together with the Behavioural Matrix, are fully completed.

Drop off the application package at the MiLAT Human Resource Office.
